team. Position: QMHP Case Manager As a Case Manager at H.U.F.
Center, you will play a critical leadership role in the daily
stabilization and progress of adults in our program. You will
provide structured psychosocial support, coordinate care, and serve
as the primary advocate for individuals working toward independence
and recovery. This is not just a job — this is purpose-driven work.
What You’ll Do ? Provide ongoing case management services to
assigned clients ? Deliver structured psychosocial and
psycho-educational interventions ? Complete comprehensive
documentation aligned with Individualized Service Plans (ISP) ?
Coordinate treatment teams and facilitate weekly case review
meetings ? Develop referral networks to ensure continuity of care ?
Complete discharge planning, monthly reports, and annual summaries
? Assist clients with ADLs/IADLs when needed ? Advocate for
appropriate services and level-of-care adjustments ? Transport
clients to community activities (after vehicle training clearance)
? Maintain professionalism while working in high-acuity

community mental health Reports to: Program Manager Requirements
Required Qualifications ? Bachelor’s Degree in Social Work or
related human services field ? Minimum 1 year documented experience
with mentally impaired or intellectually disabled populations ?
Active QMHP-A, QMHP-C, or QMHP-E certification (Virginia) ? Valid
Virginia Driver’s License ? Ability to successfully complete:
CPR/First Aid Certification Handle with Care (verbal/physical
intervention) training Annual TB screening Criminal background &
