PHYSICIAN QUALIFICATIONS Possess a Doctorate Degree in Medicine from an accredited college approved by the Liaison Committee on Medical Education and Hospitals of the American Medical Association, a Doctorate degree in Osteopathy from a college accredited by the American Osteopathic Association, or permanent certification by the Educational Commission for Foreign Medical Graduates (ECFMG). Successful completion of a residency program (corresponding to the specialty required by the task order) which has been approved by the Accreditation Council for Graduate Medical Education or the Committee on Postdoctoral Training of the American Osteopathic Association or those Canadian training programs approved by the Royal College of Physicians and Surgeons of Canada or other appropriate Canadian medical authority. Possess and maintain a current, valid, unrestricted license to practice medicine in one (1) of the fifty (50) states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands. Possess a minimum of one (1) year experience within the preceding two (2) years. Obtain Advanced Cardiac Life Support (ACLS) certification prior to contract start and maintain according to ACLS guidelines. Provide two (2) letters of recommendation written within the last two years (2) attesting to the HCW's clinical skills. A minimum of one (1) of the letters must be from a supervisor. The other letter must be from either clinic or hospital administrators or practicing physicians. Reference letters shall attest to the quality and quantity of experience including, but not limited to, the communication skills between practitioner and patient and among peers, and must include name, title, phone number, date of reference, address and signature of the individual providing reference. STAFFING AND SCHEDULING The contractor shall provide coverage Family Medicine Physicians. 4.2. Services shall be provided as stated below per week. Schedules for required shifts shall be provided to the Contractor six (6) weeks in advance. The roster of personnel shall be submitted by the contractor to the Contracting Officer's Representative (COR) four (4) weeks in advance. Personnel rosters are due on the first weekday (Monday through Friday) of the month prior to services being needed. Any changes to the personnel roster shall be submitted to the COR seventy two (72) hours in advance of the required change. The Government recognizes the unforeseen need for scheduling changes for events such as illness or injury. The required seventy two (72) hour advance notification for roster changes may be waived by the Government for such occasions on a case-by-case basis. The contractor shall make all attempts to ensure that all shifts are staffed in accordance with the personnel schedule. The HCW may not exceed the hours in the applicable Contract Line Item Number/Subject Line Item Number (CLIN/SLIN) as stated in Section B. Services required per week as follows (Part Time and PRN Options): BHC Oceana 40 hours, Monday through Friday between the hours of 0700 and 1900. Clinic is closed on Federal Holidays. TPC Virginia Beach 80 hours, Monday through Sunday between 0700 and 1900. Clinic is open on all Federal Holidays and services will be required on those days. Requirements for services may vary on holiday weeks. The government provided matrix shall reflect these changes. The Government reserves the right to change clinic hours with 30 days written notice to the contractor. The Commanding Officer or other designated government representative will supervise the HCWs. Orientation for coverage requirements. Each HCW providing service under this contract or resultant Task Orders shall undergo an orientation and shall complete mandatory Navy and DoD on-line training as required. Orientation shall be completed within 30 days following the HCW's commencement of services. Orientation may be waived for personnel who have previously provided service at the treatment facility. DoD on-line training may require that the HCW enter their Social Security Number (SSN) to document and track compliance with training requirements. Each HCW shall attend a total of 48 hours of orientation, as follows: 8 hours of Command Orientation, 24 hours of Work Space Orientation and 16 hours of additional Composite Health Care System (CHCS) & Armed Forces Health Longitudinal Technology Application (AHLTA). All training will be provided at either NMC Portsmouth, VA or the Branch Clinics. Any additional orientation shall be conducted as part of the HCW's regularly scheduled hours. SPECIFIC DUTIES. Services under this Contract/Task Order (TO) will be supporting the Navy's Medical Home Port program. The objective of this program is to enhance continuity, wellness and disease management for the patients. The HCWs WILL NOT be required to provide Obstetrics (OB) services. Provide a full range of physician services, in accordance with the core privileges as defined by BUMED Instruction 6010.30 and approved by the Commanding Officer. BUMEDINST 6010.30 Enclosure 6 details the CR requirements. A copy of BUMEDINST 6010.30 may be obtained at http://www.med.navy.mil/directives/pages/bumedinstructions.aspx. Prescribe and dispense medications as delineated by the Pharmacy and Therapeutics Committee. Provide a full range of physician services in accordance with privileges granted by the Commanding Officer. Technically guide, perform, or assist in the instruction of other health care professionals seeing patients within the scope of their clinical privileges or responsibilities. Promote preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling. Order diagnostic tests as applicable. Request consultation or referral with appropriate physicians, clinics, or other health resources as indicated. Technically guide and teach other medical staff, and provide educational lectures and participate in the provision of in-service training to clinic staff members. Such guidance and interaction will adhere to Government and professional clinical standards and accepted clinical protocols.
