/ Qualifications/n /n - Active Registered Nurse (RN) license (state
license required prior to start)/n - BLS Certification (required
prior to first shift)/n - ACLS Certification (required prior to
first shift)/n - Minimum of 2 years bedside clinical experience
(PCU, Critical Care, Step-Down, or similar)/n - Experience in PCU,
Critical Care, or Step-Down units preferred—LTAC or progressive
care experience a plus/n - Strong clinical assessment,
documentation, and patient education skills/n - Travel nursing
experience is a plus but not required/n /n/nResponsibilities/n /n -
Perform comprehensive nursing assessments within 8 hours of patient
admission and develop individualized care plans/n - Provide direct
patient care using the nursing process in an LTAC setting—critical
care and step-down skills applicable/n - Collaborate with patients,
families, and interdisciplinary teams to identify teaching needs
and establish care goals/n - Document assessments, interventions,
and patient responses accurately and in a timely manner to maintain
continuity of care/n - Participate in patient rehabilitation
programs and contribute to recovery-focused care plans/n - Initiate
discharge planning and patient education from day of admission to
support safe transitions of care/n - Communicate changes in patient
status to care team members to ensure patient safety and effective
care coordination/n - Supervise and provide direction to ancillary
nursing personnel and assistive staff/n - Promote and maintain a
safe environment for patients and staff, adhering to all safety
policies and procedures/n - Contribute positively to team building
and a supportive work environment; assume charge nurse
responsibilities as needed following orientation/n - Engage in
ongoing professional development and continuing education relevant
